  Name	Customer last name and first name associated with the service connection
  Address	Mailing address lines for customer associated with the service connection
  City	City portion of mailing address for customer associated with the service connection
  State	State portion of mailing address for customer associated with the service connection
  Class	Customer class associated with the service connection
  Size	Service connection size
  Installed	Date connection installed
  Serial #	Service connection serial number (meter, trailer, or component)
  Company Total	Total number of service connections changed out for the company
