  GUARANTOR NAME	Guarantor's First name, Middle initial and Last name
  PRIVATE BALANCE	Patient's self-responsible balance
  INSURANCE BALANCE	Patient's insurance responsible balance
  OTHER BALANCE	Patient's other balance, if applicable
  YTD SELF-PAID	Year to date payments dollar amount the patient has made
  BUDGET AMOUNT	Dollar amount the patient has agreed to pay on a regularly scheduled basis
  DATE LAST PAYMENT	Date of the patient's last payment
  TYPE	Budget type
  HOME PHONE	Patient's home phone number
  WORK PHONE	Patient's work phone number
