Field Description - Employer/Lawyer Zip

The Employer/Lawyer Zip Code (postal code) is the Zip Code associated with the mailing address for the employer or law practice.  If you enter a City Code as the Employer/Lawyer City Name, the Zip Code displays, but you can edit it if necessary.

Note:  The Employer/Lawyer Zip Code information may print in box 9c of HCFA claim forms depending on who is financially responsible for the account.

Tip:  If you don't know the Zip Code, you can look it up on the Internet.  Refer to the United States Postal Service website, http://www.usps.com/zip4/.
