The Insurance Address Line 2 is the second line of address associated with the company's mailing address if applicable. The Insurance Address Line 2 information prints in box 9c on HCFA 1500 claim forms.
Note: You have the option of printing the Address, City, State and Zip Code on one or two lines in box 9c of the HCFA form. If you elect to print the information on one line, the full address must not exceed 28 characters. The Address will be truncated if longer than 28 characters. If you choose to only print one line of Address, be sure that the Address remains legible when limited to 28 characters. For more information, contact NDS Technical Support.
