Field Description - Location Zip Code

The Location Zip Code is the Zip Code associated with the mailing address for the location.  If you enter a City Code in the Location City Name, the Zip Code displays and you can edit it.

Tip:  If you don't know the Zip Code, you can look it up on the Internet.  Refer to the United States Postal Service website, http://www.usps.com/zip4/.

Note:  The Location Address information prints in box 32 on claim forms.

Format:  up to 10 characters.  Recommendations:

• Enter the five-digit Zip Code.

• Or, enter the nine-digit Zip+4 Code with or without the dash (—).
