Provider Miscellaneous Information Maintenance

Use Provider Set Maintenance to maintain additional information an insurance carrier may require for receiving claims from a provider who performs services.  In this maintenance you can maintain the provider's professional title, first name, last name and suffix to use for billing.

• Opening Provider Miscellaneous Information Maintenance, click More.

  1.  Click File Maintenance from the Menu Bar.

  2.  Click Standard Maintenance.

  3.  Click Provider.

  4.  Click Additional from the Provider Maintenance Menu Bar.

  5.  Click Misc Provider Info.

• Creating a New Provider Miscellaneous Information record, click More.

   

  Miscellaneous Provider Info Maintenance Prompts Tip:  To see more information about a field, click the hyperlink for the field name.
  Workers' Comp Rating	Note:  Leave this field blank, unless directed by NDS.
  Workers' Comp Auth #	Note:  Leave this field blank, unless directed by NDS.
  W/C Provider Type	Note:  Leave this field blank, unless directed by NDS.
  Specialty	Note:  Leave this field blank, unless directed by NDS.
  License #	Note:  Leave this field blank, unless directed by NDS.
  Title	Enter the provider's title to appear on billing forms, if applicable of services.
  First Name	Enter the provider's professional first name to include on billing of services.
  Last Name	Enter the provider's professional last name to include on billing of services.
  Suffix	Enter the provider's suffix, if a suffix is included in the provider professional name.  The suffix will be included on services to be billed.
  HP Bill Default Proc CD	Enter the HP bill default procedure code, if applicable.
  Prevent Autobill?	Leave the default setting unless directed by NDS.
  Physician Type	Enter the physician type, if applicable.
  Supervising Physician By Form Type 1 - 9	Enter the supervising physician for each form type, if applicable.
  	To close the window after entries are finished, click Close.
  	To update the new or changed information, click Save from Provider Maintenance or press [ENTER]. Keyboard shortcut:  [ALT + s]
  	CAUTION To avoid losing data, you must save new or changed records.

   

• Editing an Existing Provider Miscellaneous Information record, click More.

  CAUTION:  To avoid losing data, you must save new or changed records.

  1.  To position the pointer to the information that you want to change, press [TAB] or click the field using the mouse.

  2.  Enter the data changes.

  3.  To complete the edit process, click Save , or press [ENTER].

• Deleting an Existing Provider Miscellaneous Information record, click More.

   

  Note:  When a provider is deleted during Provider Maintenance, the Miscellaneous Provider Information is automatically deleted.
