  Book/Seq #	The Book Number and the Sequence Number assigned to the customer.
  Account #	The Customer ID.
  Name	The Customer First and Last/Business Name.
  Class	The Customer Rate Class.
  Company	The Company ID assigned to the customer.

  Tran Code	The Transaction Code for generating the late charge transaction.
  Rate	The Rate describing the transaction code.
  Description	The full Description associated with the transaction code.
  Usage	The customer usage associated with the late charge transaction, if applicable.
  Charges	The dollar amount of the late charge transaction.
  Tax	The dollar amount of tax applied to the late charge transaction, if applicable.
  Discount	The discount dollar amount for the late charge transaction, if applicable.
  Total	The total amount of the late charge including the charge, associated tax amount and subtracting any discount amount.
