  Date Filed	Date the report is filed
  Customer ID	Customer Account Number
  Name	Customer Last/Business Name, First Name and Middle Initial
  City Placing Roll	The Name of the city or municipality placing the roll for delinquent amounts
  Address	Mailing address for the City or Municipality
  P Book	Registry of Deeds Property Book describing the customer property location
  Page	Registry of Deeds Page Number containing the property location
  Amount	Total Roll Amount for the customer
  Date Paid	Field to be written in when the customer makes a payment
  Vol	Field to be written in at the time of the customer payment
  Page	Field to be written in at the time of the customer payment
  Line	Field to be written in at the time of the customer payment
