If you entered a provider code as the Referred To Provider, the Form Type Provider Print Code displays and can be edited.
Or, if you entered a Referral Source Code as the Referred to Provider, the Referral Form Type UPIN displays and can be edited.
Or, if you entered a provider name as the Referred to Provider, you can enter the Provider's state license number or Personal Identification Number (PIN).
