Charge/Payment Combo Journal Sample

The Charge/Payment Combo Journal lists the charge and payment transactions entered for the day.  Use the journal to verify the entries and to provide an audit trail of the day's activities.  In addition, you can use the journal to monitor the percentage of charges that were collected at the time of service by matching payments directly to services.

  ACCT#	Patient account number
  PATIENT/INSURED	Patient first name, middle initial and last name
  TICKET#	Number of patient ticket assigned to the service or the payment
  SERVICE/ PMT DATE	Date of Service or date of payment
  PROC./ TRANS	Procedure code for the service performed or the Transaction code that identifies the type of payment
  SERVICES	Dollar amount of patient's service
  PAYMENT	Dollar amount of payment
  ADJ/REF	Dollar amount of adjustment or refund
  PREVIOUS A/R	Previous accounts receivable dollar amount for the patient
  NEW A/R	New accounts receivable dollar amount the patient is responsible to pay Calculation:  Previous A/R + Services - (Payments + Adjustment)
