  Emp #	Employee Number.
  Employee Name	Employee First Name and MI, Employee Last Name.
  Employee MTD Hrs/Dys	Month-to-date MSR Expected Hours or Days per Week.
  Employee Ret MTD	Month-to-date MSR Retirement Amount.
  Employee Ret QTD	Quarter-to-date MSR Retirement Payback Amount.
  Employee Basic	M-T-D Basic Insurance Withholding Amount.
  Employee Suppl	M-T-D Supplemental Insurance Withholding Amount.
  Employee Dep	M-T-D Dependent Insurance Withholding Amount.
  Employer Basic	Employer's Basic Insurance Premium.
  Employer Suppl	Employer's Supplemental Insurance Premium.
  Employer Dep	Employer's Dependent Insurance Premium.
  Total Ins	Total Insurance Premium for the employee.
