Electronic Billing Receive Log Sample

The Receive Logs are the reports the insurance carrier sends back to you confirming the claims information that they received during the electronic billing session.

Although the receive logs for each insurance carrier have a slightly different format, in general each carrier provides the following information:

• Acknowledgement Report or 997 Report, also known as the Level 1 Report - Reports whether the insurance carrier's electronic billing file as a whole is accepted or rejected.  The Acknowledgement Report interprets the 997 Report in a simple, easy to read format and tells whether the file has been accepted or rejected as a whole.

• Receive Log or Error Summary Report, also known as the Scrubber Report (Medicare), Level 2 Report (Anthem) or Receive Log - Reports each claim in the electronic billing file and whether the claim is accepted or deleted (which means the claim failed) and the reason why it failed.

• Little p Report - Reports information pertaining to your Medicare Send session and reports the number of claims they received.  This report only pertains to Medicare.

Acknowledgement Report

  1.  To see further details for a specific error message, click the hyperlink.

  2.  A new window opens displaying the invalid billing information for your review.  An example is shown below.

  3.  You will need to make the necessary corrections to the data and then re-submit the file by starting over at the Setup step.

  4.  When you are done viewing the information click in the upper-right pane to return to the Acknowledgement Report.

   

  

   

Anthem Receive Log or Level 2 Report

  Provider ID #	Provider Identification
  ISA Control #	ISA Control Number which should match the electronic billing print journal Group and Transaction Set Number
  St Control #	Electronic billing number in the electronic billing file
  Provider Name	Provider Name
  Batch Control	Number of batches that should match the electronic billing print journal
  Status Control	Electronic billing submission status code determined by the insurance carrier
  Patient Name	Patient's Last Name, First Name and Middle Initial
  Subscriber ID	Insurance Policy Holder Member ID
  Date of Service	Date of service for charge
  Total Charge	Dollar amount of charge
  Payer Document Control #	Payer Document Control Number assigned by the insurance carrier
  Account	Patient Account Number
