  MISSING BILLING INFORMATION	Displays the name of the insurance company
  Note:  For the insurance company listed above, the following fields display to list the information that is missing or incorrect.
  FILE	Name of the file containing the missing or incorrect billing information
  KEY	Patient Account Number
