Field Description - Patient Guarantor Last Name

The Patient Guarantor Last Name is the last name of the person who is the guarantor of the patient's account.  The Patient Guarantor First Name and Initial combined with the Patient Guarantor Last Name makes up the guarantor's full name.

Notes:  

• The guarantor must first be set up as a patient, even if they are not a patient and are only paying the bills.

• The guarantor's full name and address print on patient statements.
