Open
the Demographics Window.
Tip: For
details about opening windows, refer to the Patient
Reference Panel.
Click the Additional Address Information icon .
Patient Address Maintenance Window Example:
To create a new record choose one of these methods:
Enter
the new Address ID and click Retrieve , or press
[ENTER].
Keyboard shortcut: [ALT
+ t]
Note: The
program automatically assigns the next numeric Address ID available if
you leave the field blank and click Retrieve.
Or,
enter the new Address ID and click
File from the Menu
Bar; next click New.

Tip: To indicate that you are building a new Patient Address record, New displays near the Address ID. Default entries display and the first field is highlighted to let you know you can begin entering information.

Enter a unique code that identifies the address record for the patient. You can create multiple address records for a patient. (up to 5 characters) Tips:

To update the new or changed information, click Save or press [ENTER] if the button is in focus. Keyboard shortcut: [ALT + s] | |
CAUTION To avoid losing data, you must save new or changed records. | |
To view all of the existing Patient Address records for the open patient, click Search. From the Search list you can select the record to change or delete. Keyboard shortcut: [ALT + r] | |
To display an existing Patient Address record, enter the Address ID and click Retrieve, or press [ENTER] if the button is in focus. Keyboard shortcut: [ALT + t] | |
Enter the title used to address the person included in this patient address record. (up to 15 characters) Examples: MR, MS, MISS, MRS | |
For your convenience when entering a new patient address record you can click one of the Name Source buttons to load in the name from the source indicated by the button. Buttons:

Enter the First Name for the person included in this patient address record. (up to 25 characters) | |
Enter the Middle Name or Initial for the person included in this patient address record. (up to 15 characters) | |
Enter the Last Name for the person included in this patient address record. (up to 35 characters) | |
Enter the Suffix for the name of the person included in this patient address record. (up to 8 characters) Examples: MD, JR, SR, PHD | |
Enter the Address for the person included in this patient address record. (3 lines, up to 36 characters) | |
Enter the city name, town name or a valid City Code (to fill in the associated City, State and Zip Code for this person). (up to 15 characters) Notes:

The US Postal Service State Code displays if you entered a City Code above. Otherwise, enter the US Postal Service State Code for this city. (2 characters) | |
The Zip Code displays if you entered a City Code above. Otherwise, enter the five-digit Zip Code or the nine-digit Zip+4 Code. (up to 10 characters) | |
Enter the Country Name or Country Code if the address is for a person in a foreign country. (up to 10 characters) Note: To search for a Country, click Search or press [CTRL + f]. | |
Enter the primary phone number for the person included in this patient address record. (up to 10 characters) | |
Enter the work phone number for the person included in this patient address record. (up to 10 characters) | |
Select one of the following Address Types in the drop-down list to specify how to use this address record.

To print this address on statements, select (Y)es. To omit this address from statements, select (N)o. | |
To print this address on 1500 forms, select (Y)es. To omit this address from 1500 forms, select (N)o. | |
To print this address on UB-92 forms, select (Y)es. To omit this address from UB-92 forms, select (N)o. | |

Enter the date on which the person started occupying the location in this address. (mmddccyy) | |
Enter the date on which the person stopped occupying the location in this address. (mmddccyy) | |
To complete your entries, close this window and return to the main Patient Entry Window, click . |

you do not know the Address ID, you can click Search to see a list of Patient Addresses for the open patient.
To position the pointer to the information that you want to change, press [TAB] or click the field using the mouse.
Enter the data changes.
To
complete the edit process, click Save , or press [ENTER]

To
display the record to be deleted, enter the Address
ID and click Retrieve , or press [ENTER]

Tip: If
you do not know the Address ID, you can click Search to see a list of Patient Address records for the open patient.
Click
File from the Menu
Bar, and next click Delete.
Keyboard shortcut: [ALT]

At
the Delete Records prompt, click Proceed
to delete the record, or click Cancel
to escape from the delete routine.

Use Patient Address Maintenance to maintain additional addresses for the patient in the Patient Entry - Demographics Window. The addresses can apply to the patient, guarantor, policy holders, or some other person depending on the Address Type. The table following the window example describes the Patient Address Maintenance window.
