Patient Guarantor Wizard - Overview
Use the Patient Guarantor Wizard, which is launched from other Patient Wizards, to quickly and easily create or edit guarantor demographic and insurance information, when the guarantor is not the patient. The guarantor is the person responsible for the patient's account, is usually the holder of the insurance policy, and receives the private statements. For example, a parent is often the guarantor for a minor child. In INSight a guarantor must have a patient record, even if not actually a patient of the practice. The Wizard guides you through each step of data entry in an organized manner. It also provides the flexibility to skip around among the steps. The table below describes each step for the Patient Guarantor Wizard.

Contains the Patient Account Number. |

Contains the Social Security Number to confirm that you are entering a patient that does not already exist. Note: This step does not appear if the guarantor already exists. |

Contains the patient's name, address, marital status and other demographic information. |

Contains patient contact information such as the cell phone number, fax, email, and a contact person. |

Contains additional patient information such as the employment status, student status, consent code and organ donor code. |

Contains three user-defined comments fields. |

Contains up to three diagnosis codes for the patient. |

Contains disability and status information for patients with Workers' Compensation claims. |

Contains fields defined by the Workers' Compensation Control Record. There are three date fields and eight alphanumeric fields that you can tailor to your practice's needs. |

Contains patient-specific remarks used for billing purposes. |

Contains the patient's Provider and Referral Codes, if applicable. |

Contains employer information for the patient, which is needed if the patient has a workers' compensation related illness or injury. |

Contains the list of assigned insurances for the patient. Launches the Patient Insurance Wizard. |

Allows you to view or remove existing images from the patient, such as scanned medical cards or the patient's photo ID. |
