Patient Entry - Name & Address Area

Use the Name & Address Area to maintain the patient's name, address and phone number information.  The table following the window example describes the Name & Address Area for the Patient Entry - Demographics Window.

Tip:  You can create additional addresses for a patient, guarantor, policy holder, or other using Patient Entry - Patient Address Maintenance.

Directions:

To display the Name & Address Area, open the Demographics Window.

Tip:  For details about opening windows, refer to the Patient Reference Panel.

Tip:  To see more information about a field, click the hyperlink for that field.

Last Name

Enter the patient's Last Name.

Or, to set up family members, you can enter a valid patient account number here to automatically fill in the associated patient information in the Last Name through Phone Number fields, excluding the First Name field. (up to 20 characters)

First Name, Init

Enter the Patient's First Name, a space and their Middle Initial. (up to 18 characters)

Address 1

Enter the first line of the patient's mailing address. (up to 25 characters)

Address 2

Enter the second line of the patient's mailing address, if applicable. (up to 25 characters)

City

Enter the city name, town name or a valid City Code (to fill in the associated City, State and Zip Code for this patient). (up to 15 characters)

Notes:  

  • To search for a City Code, click Search or press [CTRL + f].

  • To create a new City Code, click Fast Maintenance or press [CTRL + o].

State

The US Postal Service State Code displays if you entered a City Code above.  Otherwise, enter the US Postal Service state code for this city. (2 characters)
Required

Zip

The Zip Code displays if you entered a City Code above.  Otherwise, enter the five-digit Zip Code or the nine-digit Zip+4 Code. (up to 10 characters, dashes (-) not necessary)

To see where the patient resides in order to provide directions to your practice, click Web Map Search.

Phone Number

Enter the patient's Phone Number. (up to 10 characters)

If patient statements or other correspondence is undeliverable because of an invalid address, click Patient Search to load Web People Search where you can verify patient address and phone number information.

Note:  Do not confuse Web People Search with Search described above.  They are 2 separate functions.
