Patient Address Maintenance Prompts

Tip:  To see more information about a field, click the hyperlink for the field name.

Address ID

Enter a unique code that identifies the address record for the patient.  You can create multiple address records for a patient.  (up to 5 characters)

Tips:

• Although the system can automatically assign the next numeric ID, you can also enter alphanumeric IDs.

• You can use the same ID for different patients.  For example, Bob Smith and Sally Walton can both have patient addresses for IDs 00001, 00002, and 00003.

To update the new or changed information, click Save or press [ENTER] if the button is in focus.

CAUTION

To avoid losing data, you must save new or changed records.

To view all of the existing Patient Address records for the open patient, click Search.  From the Search list you can select the record to change or delete.

Keyboard shortcut:  [ALT + r]

To display an existing Patient Address record, enter the Address ID and click Retrieve, or press [ENTER] if the button is in focus.

Keyboard shortcut:  [ALT + t]

Title

Enter the title used to address the person included in this patient address record.  (up to 15 characters)

Examples:  MR, MS, MISS, MRS

For your convenience when entering a new patient address record you can click one of the Name Source buttons to load in the name from the source indicated by the button.

Buttons:

• P - Patient

• G - Guarantor

• P1 - Policy Holder 1

• P2 - Policy Holder 2

• P3 - Policy Holder 3

• P4 - Policy Holder 4

• P5 - Policy Holder 5

• P6 - Policy Holder 6

First Name

Enter the First Name for the person included in this patient address record.  (up to 25 characters)

Middle

Enter the Middle Name or Initial for the person included in this patient address record.  (up to 15 characters)

Last Name

Enter the Last Name for the person included in this patient address record.  (up to 35 characters)

Suffix

Enter the Suffix for the name of the person included in this patient address record.  (up to 8 characters)

Examples:  MD, JR, SR, PHD

Address

Enter the Address for the person included in this patient address record.  (3 lines, up to 36 characters)

City

Enter the city name, town name or a valid City Code (to fill in the associated City, State and Zip Code for this person). (up to 15 characters)

Notes:  

• To search for a City Code, click Search or press [CTRL + f].

• To create a new City Code, click Fast Maintenance or press [CTRL + o].

State

The US Postal Service State Code displays if you entered a City Code above.  Otherwise, enter the US Postal Service State Code for this city. (2 characters)

Zip

The Zip Code displays if you entered a City Code above.  Otherwise, enter the five-digit Zip Code or the nine-digit Zip+4 Code. (up to 10 characters)

Country

Enter the Country Name or Country Code if the address is for a person in a foreign country.  (up to 10 characters)

Note:  To search for a Country, click Search or press [CTRL + f].

Home Phone

Enter the primary phone number for the person included in this patient address record.  (up to 10 characters)

Work

Enter the work phone number for the person included in this patient address record.  (up to 10 characters)

Address Type

Select one of the following Address Types in the drop-down list to specify how to use this address record.

• Patient

• Guarantor

• Policy Holder 1

• Policy Holder 2

• Policy Holder 3

• Policy Holder 4

• Policy Holder 5

• Policy Holder 6

• Other

Statements

To print this address on statements, select (Y)es.

To omit this address from statements, select (N)o.

1500s

To print this address on 1500 forms, select (Y)es.

To omit this address from 1500 forms, select (N)o.

UB-92s

To print this address on UB-92 forms, select (Y)es.

To omit this address from UB-92 forms, select (N)o.

Institutional

To print this address on Institutional forms, select (Y)es.

To omit this address from Institutional forms, select (N)o.

Professional

To print this address on Professional forms, select (Y)es.

To omit this address from Professional forms, select (N)o.

Active

To specify that this address is active, select the box so it is checked.

To specify that this address is inactive, clear the box so it is unchecked.

Seasonal

To specify that this address is occupied seasonally, select the box so it is checked.

To specify that this address is not occupied seasonally, clear the box so it is unchecked.

From Date

Enter the date on which the person started occupying the location in this address.  (mmddccyy)

To Date

Enter the date on which the person stopped occupying the location in this address.  (mmddccyy)
